[1099-MISC] Incorrect X marks and missing/merged values in 2-Up and 4-Up print forms for recipients

1. In the 4-Up form, the X mark for the FATCA filing requirement is missing even though the option is selected for the recipient.& 2nd,TIN filed also missing
2. In the 4-Up form, an unnecessary X mark is displayed in the 13a field.
