[1099-MISC] Incorrect X marks and missing/merged values in 2-Up and 4-Up print forms for recipients.

Steps to Reproduce:-
1. Create a recipient and enter values in all applicable fields.
2. Navigate to File Menu click on Print Forms for Recipients.
3. Generate and print the 2-Up and 4-Up forms.
4. Open the generated PDF.
5. For the 4-Up form, observe the FATCA filing requirement and 13a fields.
6. For the 2-Up form, observe the values in the 13th, 13b, and 14th fields.

Actual Result:-
1. In the 4-Up form, the X mark for the FATCA filing requirement is missing even though the option is selected for the recipient.
2. In the 4-Up form, an unnecessary X mark is displayed in the 13a field.
3. In the 2-Up form, the values entered in the 13th and 14th fields are merged together in the generated PDF.
4. In the 2-Up form, the 13b field value is not displayed in the generated PDF.
