[Employer] - 1094B Employer Screen UI Issues.

#1.Issuer or Other Coverage Provider Section - Stat or Province Dropdown is adjusted to down and causing a slight alignment issue.

#2.Employer Information Section - All field boxes are not adjusted in a straight order. Once refer the 2025 application.

#3.Adjust the "Address not in US Territory" Checkbox upto some extent upwards. Currently is very closed to the employer name field.
