Create a company with all mandatory details.
Navigate to the MISC recipient form.
Enter all required details, including the newly added fields 13a and 13b. Save the MISC form.
Select Print Form for Recipients and generate the 2UP print form for the MISC recipient.

Actual Result
The newly added fields 13a and 13b are not displayed in the generated 2UP print form.

Expected Result
The generated 2UP print form should include the values entered for the newly added fields 13a and 13b.

Issue#2: The 14th field, overtime compensation saved data also missing in the 2up forms.
NOTE: The above issues exist in both 2up forms of recipients and filing.
