The fields have been updated; however, the following corrections are still needed:

2a. Total gross proceeds – Please verify/update the text.
2b. Cash gross proceeds – Please verify/update the text.
Field 6 – The text "of the consideration." is missing.
Field 8c – The text "received, as consideration" is missing.
Alignment – Please fix the alignment issues for the updated fields.
